I’m in two minds when someone asks me if I’m ok. Part of me want’s to say “of course not, do I look like I’m fine?” The other part of me keeps thinking  “wow, someone noticed I’m not ok, maybe I’m not so worthless.” Opinions?

Put both minds together for a joint effort in cognition. Ultimately it’s up to you to decide which answer you give, but BE ADVISED that somewhere out in the “big-scary-omg-stahp” world there are exceptional individuals who will take a genuine interest in your welfare and you as a person.
Most of the time I will just lie and say I am Ok. On the odd occasion and with the right person I will just tell it how it is
