I don’t think that most state in the US have standardardized forms for advance directives. I downloaded mines from a third-party online. Just type in “[statename] advance directive forms” into google and you should get something.
Good move with the advance directives btw. Regardless of your intentions (I filled out mine because I was planning on shooting myself and I didn’t want doctors to revive me), they are good to have.
There are some issues with advance directives, like people who attempt suicide in a fashion which wont kill them but could potentially severely disable them without medical intervention (I’m talking like ODs, falling a relatively short height etc etc). Do you intervene in these cases, where you know someone will not die but you’ve been asked not to interfere?
Gotta take the above into account when writing up your document. I don’t know about you, but if I am not successful, I sure as hell do not want to be disabled to the point where I cannot attempt again. Cheers.

Oh, and register your advance directive with state government if you can. CA allows this for sure; don’t know about other states.
